Eyehealth Form

Realeyes is an education program teaching students about eye health and safety through a program that meets Ohio academic standards. It currently includes four interactive, age-appropriate curricula for you to choose from, ALL FREE. Typically Realeyes is taught by a local optometrist in the classroom, but this year it is being offered for students to learn from home with pre-recorded presentations for ages grades PreK-12th. Each curriculum includes an entertaining video and downloadable pdf packet of activities. Fill out the form below to have access to the programs created by the Ohio Optometric Association through a grant from the Ohio Department of Health Save Our Sight Fund. Realeyes has educated 1.3 million students in Ohio since 2000 and receives rave reviews from teachers and schools.
